In addition to the above insurance carriers, we accept most commercial insurances. If in doubt, please contact our office for information.
Although we are not listed on a Geisinger card as a participating laboratory, please be assured that we do participate in Geisinger's network. The same holds true for Aetna and Health America or any other HMO plan.
FPH Non-Par referrals can be obtained by filling out the Non-Par request form and submitting to the fax number listed on the form on the DATE OF SERVICE. Please send the specimen on the day it is obtained to insure a fast turnaround time. Effective January 1, 2009, NaviNet-enabled offices can submit requests electronically on the web site. You can also call provider relations at 1-800-962-5353 to assist you in the Non-Par referral request process.
